NEW Account Conversion Checklist
To Ensure a smooth closing of previous bank's checking account:

| Cease transactions with previous bank's checking and/or savings account(s). | Customer | Immediately |
| Make sure all transactions, have cleared previous bank's checking and/or savings account(s). | Customer | Immediately |
| If outstanding transactions, keep funds in the account at previous bank to honor these debts. | Customer | Immediately |
| Continually monitor the progress of account(s) conversion with customer. | CSR | Ongoing until new account relationship has been established |
Changing/Establishing Authorization for Direct Deposit and Automatic Payments:
