NEW Account Conversion Checklist

To Ensure a smooth closing of previous bank's checking account:

Complete Account Closing Authorization and submit to previous financial institution.

Cease transactions with previous bank's checking and/or savings account(s).

Make sure all transactions, have cleared previous bank's checking and/or savings account(s).

If outstanding transactions, keep funds in the account at previous bank to honor these debts.

Continually monitor the progress of account(s) conversion with customer.

Changing/Establishing Authorization for Direct Deposit and Automatic Payments:

ACTION WHO WHEN

Notify employer of Direct Deposit change to Lincoln Savings Bank (Authorization Agreement for Direct Deposit).

Immediately

Identify sources of Automatic Payment (review customer bank statement from previous financial institution).

Immediately after identification of vendors

Complete Automatic Payment Change Request and mail to appropriate vendors.

If desired, complete Automatic Payment Authorization to
establish new debit relationship with outside vendor.
